Step 2- Submit IACE Travel Forms

After the students have been selected, and the TEACHER has completed ALL of Step 1, IACE may then obtain a ProgettoScuole username on the school's behalf. ProgettoScuole is an internet based portal intended for learning the Italian language. Final projects will be posted on this site. (For more information see Step 3)

In anticipation of the username, please have all parents participate by helping the the candidates complete and send to IACE ALL of the following by the deadline :

• 3 copies of a valid U.S.A. passport - Please note that, if the student does not have a U.S.A. passport, it is the responsability of the parents and teachers to determine the appropriate travel documents that are required for travel abroad (examples: green card, visas etc.).
• Parent Authorization
• Assumption of Risk and Waiver
• Health History
• 2 copies of the student's Health Insurance card or other valid proof of Health Insurance documentation  
• Release Form

All of the above must be submitted by May 25^th, 2010.  Students will not be able to particpate without  the ProgettoScuole Username.
